Mother & Baby App Launched

The mum & baby app launched in November 2018 and includes innovative features

1) self-referral can be done as soon as the woman is aware of her pregnancy

2)enabling women and their families easy access to accurate and up-to-date local maternity services information to confidently make choices about their maternity care provider, birth setting and other care options;

3) providing easy access to clinician-validated and locally pertinent information, which has been collaboratively approved, thus addressing the challenge of inconsistencies in information;
4) encouraging women and their families to actively participate in their maternity care by being prompted to co-create and share their digital care plans with the clinical teams.

The app is free to use and easy to download from the App Store or Google Play

Primary Care Network launched

Primary care networks build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care. Clinicians describe this as a change from reactively providing appointments to proactively care for the people and communities they serve. Where emerging primary care networks are in place in parts of the country, there are clear benefits for patients and clinicians.
